Bank Street Dental Plans - terms & conditions
  1. Introduction
    1. These Terms and Conditions set out the agreement between you (the Member) and Bank Street Dental Practice (the Provider).
    2. By enrolling in this dental plan, you agree to comply with these Terms and Conditions.
  2. Membership and Eligibility
    1. Membership is available to individuals who meet the eligibility criteria as determined by Bank Street Dental Practice.
    2. For new patients to the Practice, A dental assessment will be required, which will incur a fee.
    3. Membership is non-transferable and applies only to the registered member.
    4. Regular dental and hygiene appointments (every six months) will be available as part of the plan after four monthly payments have been collected.
  3. Payment and Fees
    1. Membership fees are payable monthly or annually as agreed upon registration.
    2. Payments must be made via direct debit, credit/debit card, or other approved methods.
    3. Failure to make payments may result in suspension or cancellation of the plan.
    4. Fees are reviewed annually, and members will be given prior notice of any changes.
  4. Covered Services
    1. The plan covers routine preventive care, including:
      Regular dental check-ups (e.g., every six months)
      Scale and polish/hygiene appointments
      Routine X-rays- Bitewings two bi-annually
      Preventive treatments (e.g., fluoride application)
    2. The plan may also include:
      Accidental dental trauma
      5% discount on private dental treatment (see exclusions and limitations)
      Worldwide emergency dental cover (if applicable)
    3. A full list of covered services is available in the membership documentation.
  5. Exclusions and Limitations
    1. The plan does not cover:
      Cosmetic treatments (e.g., teeth whitening, veneers)
      Orthodontic treatments
      Dental implants and specialist procedures
      Treatments required due to pre-existing conditions (unless otherwise stated)
      Treatments received from non-approved providers (except in emergencies)
    2. Emergency dental treatments abroad may be covered up to a set limit.
  6. Appointments and Cancellations
    1. Members must schedule and attend regular check-ups to maintain coverage.
    2. Missed appointments may incur a charge.
    3. Members should provide at least 24 hours’ notice for cancellations.
  7. Changes to the Plan
    1. Bank Street Dental Practice reserves the right to modify or discontinue the plan with prior notice.
    2. Members will be informed of any significant changes in coverage or fees.
  8. Membership Cancellation and Termination
    1. Members may cancel their plan by providing 30 days’ written notice.
    2. Refunds will not be provided for partial months of membership.
    3. Bank Street Dental Practice may terminate membership if:
      Payments are not made on time
      A member violates the Terms and Conditions
      A member provides false or misleading information9.
  9. Complaints and Dispute Resolution
    1. Complaints should be submitted in writing to Bank Street Dental Practice at [email protected].
    2. The practice aims to resolve complaints within 28 business days.
  10. Data Protection and Privacy
    1. Bank Street Dental Practice will process personal data in compliance with data protection laws.
    2. Members’ information will not be shared without consent, except as required by law.
  11. Liability and Legal
    1. Bank Street Dental Practice is not liable for any indirect or consequential losses.
    2. These Terms and Conditions are governed by the laws of the United Kingdom.
    3. By enrolling in the Bank Street Dental Practice Plan, you confirm that you have read, understood, and agree to these Terms and Conditions.
